Pomodoro for ADHD: what to change about the break

Two things before anything else, because this subject attracts a lot of confident nonsense. This is not medical or psychological advice, it is not treatment, and nothing here replaces working with a clinician. It is an article about the mechanics of a timer, written by people who make one.

There is also no research we can point you at showing that Pomodoro timers, or movement breaks, do anything specific for ADHD. Not a weak trial, not a small one. Nothing. Anyone telling you otherwise has either not looked or is hoping you will not. What follows is a structural argument plus the general evidence that does exist, clearly labelled as such.

The standard advice fails in a predictable place

The usual pitch is that a Pomodoro timer helps you start. Set 25 minutes, work, stop. And starting is a real problem, so the pitch lands.

But in practice the thing that ends most attempts is not the first interval. It is the fourth break. You stop at 25 minutes, pick up your phone, and rejoin the world ninety minutes later having intended to be gone for five.

That failure is entirely predictable from the design, and it has nothing to do with discipline. An empty break is an unstructured interval with no defined end and an infinite menu of things that are more immediately interesting than what you were doing. You do not need any particular diagnosis to lose an afternoon to that. You just need it to happen slightly more easily than average.

So the argument of this article is narrow: the break is the part worth redesigning, and the standard advice barely mentions it.

Stopping is a transition, and transitions are the expensive part

The Pomodoro technique asks you to change state four times an hour. Work to break, break to work, over and over. Every one of those is a transition, and transitions are where the friction lives, in both directions.

Getting started is the transition everyone talks about. The other one gets ignored: stopping something you are finally deep in, on a bell, because the clock says so. That is genuinely unpleasant, and the usual advice is to obey the bell anyway, which is how people end up resenting the timer and abandoning it in week two.

There is a second-order version of this too. If you know a break is coming that will be hard to come back from, part of you will resist stopping. The break’s shapelessness makes the work harder to leave. Fix the break and the boundary gets cheaper in both directions.

What a movement break does mechanically

Set aside physiology for a moment. Structurally, putting a specific exercise in the break changes three things.

  • It has a defined end. Forty-five seconds of air squats finishes. A scroll does not finish. That is the single most important difference, and it is a property of the activity rather than of your willpower.

  • It removes a decision at the worst possible moment. You have just stopped concentrating. That is not the point at which to open a menu. One movement on screen with a countdown already running asks nothing of you.

  • It changes the room. You stand up, you are physically somewhere else, and coming back to the desk is a deliberate act rather than the continuation of a drift.

None of that is a claim about attention. It is a claim about how the five minutes is shaped, and shape is the thing the standard advice leaves out.

What the evidence actually supports, in general populations

Everything here is from studies in general or student populations. None of it is specific to ADHD, and we are not going to pretend it transfers cleanly.

Movement lifts energy and mood, reliably. The largest pooled estimate here draws on 158 studies totalling 13,101 people. Acute aerobic exercise raised activated positive affect at d=0.47, an effect still present half an hour later, and a low effort was sufficient. This is the best-supported claim in the entire category.

Movement does not improve thinking. Several trials interrupted sitting with walking, then tested executive function and memory, and found nothing. Reviewers pooling 113 studies in 2024 arrived at g=0.13 for exercise’s short-term effect on thinking, and flagged strong indications that null findings had gone unpublished. If somebody tells you squats will improve your working memory, that is the study they are misreading. The full version is in do exercise breaks improve concentration.

The 25-minute interval is not a research finding. Francesco Cirillo settled on it in the late 1980s with a kitchen timer shaped like a tomato. Nothing in the research produced that number, and the technique has attracted only about four peer-reviewed studies in total.

Those studies are not flattering. In a 2025 randomised trial of 94 students, a 25/5 pattern produced no measurable advantage in productivity, in tasks completed or in flow over Flowtime or self-chosen breaks, and the Pomodoro group tired sooner and lost motivation sooner than the self-regulating one. In an earlier study of 87 students, scheduled breaks helped mood, yet mental effort and the amount of work finished did not differ measurably.

One finding does support interruption, with a caveat. In 2011, Ariga and Lleras found that short diversions to an unrelated task about every ten minutes held performance steady through a 40-minute vigilance task, where each control condition fell away. The switch there was mental, not physical, and a vigilance task is not your inbox. Treat it as suggestive.

Put together, the defensible position is that a timer is a structure rather than a stimulant. It does not make your attention work differently. It moves some decisions out of the moment when deciding is hardest.

And the thing we are not going to say

You will see a great deal of writing that explains all this with dopamine. Movement releases dopamine, ADHD involves dopamine, therefore movement breaks are uniquely effective for ADHD.

We are not making that argument. It is a chain of plausible-sounding steps with no trial at the end of it, the underlying neuroscience is far messier than the summary suggests, and the mechanism story is doing work the evidence has not done. Being told a satisfying reason why something should work is not the same as being shown that it does.

The structural argument above stands on its own and requires no neurochemistry at all.

A practical structure

Adjust freely. The intervals matter less than having decided them in advance.

  1. Make the work block match the task, not the tradition. Twenty-five minutes suits short, self-contained things. For anything that needs context loaded, 45 or 50 is more honest, and stopping at 25 mid-untangle is the fastest way to hate the method.

  2. Decide the break’s contents before the break. This is the whole point. A movement already chosen, on screen, with its own countdown.

  3. Keep it bounded and physical. Air squats, calf raises, desk-edge push-ups, a walk to the end of the hall and back. Anything with a screen in it is a different break and it does not end.

  4. Allow yourself to skip, deliberately. If you are mid-flow, keep going. A method that requires perfect compliance is a method you will fail at once and then discard.

  5. Aim for a rhythm, not a streak. Roughly hourly movement is what most people sustain. Across more than eleven thousand desk workers surveyed in 2026, an hourly cadence delivered nearly what a half-hourly one did, and people stuck with it far more readily.

  6. Do not gamify it. Points and streaks turn one missed day into a reason to stop entirely. That is a design choice a lot of focus apps make and it is worth avoiding.

If deciding all of that is itself the obstacle, the ready-made plans do it for you. Weekly brain fog killer suits an afternoon. Early morning focus suits people whose good hours are early.

When this is the wrong tool

Written plainly, because the honest version is more useful than the sales version.

  • If a countdown running while you work is itself distracting, this method is adding a problem. Some people find a visible clock genuinely intrusive.

  • If being interrupted mid-flow is your main cost, use longer blocks or a different structure entirely. Flowtime and other patterns exist for exactly this.

  • If the exercise reads as one more thing to fail at, skip the movement and use a plain timer. That is a real response and it is worth respecting.

  • If your day has no blocks in it, because it is meetings or support work, there is nothing for a timer to count.

  • If you are looking for treatment, this is not it and cannot be. A timer is a piece of furniture for your attention, not a clinical intervention.

The honest summary

A Pomodoro timer is a structure. It decides in advance when to stop, and putting a specific movement into the break gives that break an end, which is the property it usually lacks. Everything else in this article is either general-population evidence or an argument about design.
